Effects of chronic disease trajectory framework model intervention on disease cognition and self-management ability of elderly patients with diabetes mellitus complicated with pancreatitis

Abstract:Objective To explore the effects of chronic disease trajectory framework model intervention on disease cognition and self-management ability of elderly patients with diabetes mellitus complicated with pancreatitis. Methods Taking the intervention regimen based on chronic disease trajectory framework model implemented in Ganzi Tibetan Autonomous Prefecture People’s Hospital as the dividing line, 61 elderly patients with diabetes mellitus and pancreatitis who underwent routine intervention from July 2020 to June 2021 were included in control group, complicated with another 61 elderly patients who received chronic disease trajectory framework model intervention from July 2021 to June 2022 were enrolled as observation group. Disease cognition, self-management ability, quality of life were compared before and after three months of intervention. Results After three months of intervention, the scores of disease cognition (disease perception, emotional state, and disease understanding degree) in the two groups were decreased compared with those before intervention, and the various scores in observation group were significantly lower than those in control group (P<0.05). The scores of self-care ability (self-concept, self-responsibility, health knowledge level, and self-care skills) and quality of life (physical health, mental health, social function, and material life) were enhanced compared to before intervention, and the above scores in observation group were higher than those in control group (P<0.05). Conclusion Chronic disease trajectory framework model intervention can improve the disease cognition, enhance the self-management ability, and increase the quality of life of elderly patients with diabetes mellitus complicated with pancreatitis.
