中药周期疗法联合二甲双胍治疗体重正常型多囊卵巢综合征的效果及对性激素、胰岛素抵抗的影响

摘要目的探讨中药周期疗法联合二甲双胍治疗体重正常型多囊卵巢综合征（PCOS）的效果，分析对性激素、胰岛素抵抗的影响。方法选择2016年4月~2017年12月于北京中医药大学东直门医院诊治的90例体重正常PCOS患者为研究对象，采用随机数字表法分为两组，研究组（45例）采用中药周期疗法联合二甲双胍治疗3个月，对照组（45例）采用二甲双胍治疗3个月。比较两组疗效和安全性，并分析治疗前后性激素、空腹血糖（FPG）、空腹胰岛素（FINS）、胰岛素抵抗指数（HOMA-IR）、排卵率的变化。结果治疗后两组月经稀发、闭经、腰酸、怕冷、性欲冷淡、多毛、小便清长、大便稀烂症状的临床症状积分均出现下降（P ＜ 0.05），且研究组上述积分均低于对照组（P ＜ 0.05）。治疗后两组后雌二醇（E2）、排卵率均有升高（P ＜ 0.05），且研究组高于对照组（P ＜ 0.05）；治疗后研究组促黄体生成素（LH）、睾酮（T）、LH/卵泡刺激素（FSH）、FPG、FINS、HOMA-IR降低（P ＜ 0.05），对照组LH、FPG、FINS、HOMA-IR降低（P ＜ 0.05），T、FSH、LH/FSH无变化（P ＞ 0.05）。治疗后研究组LH、T、LH/FSH、FPG、FINS、HOMA-IR低于对照组（P ＜ 0.05）。研究组治疗有效率高于对照组（P ＜ 0.01），不良反应发生率低于对照组（P ＜ 0.05）。结论中药周期疗法联合二甲双胍治疗体重正常型PCOS效果显著，安全性更高，可显著降低PCOS患者性激素和胰岛素抵抗水平。

Abstract：Objective To explore the effect of traditional Chinese medicine cycle therapy combined with metformin in the treatment of normal-weight polycystic ovary syndrome (PCOS), and analyze the effects on sex hormones and insulin resistance. Methods From April 2016 to December 2017, Dongzhimen Hospital of Beijing University of Chinese Medicine, 90 patients with normal weight PCOS were selected, they were divided into two groups by random number table method. The study group (45 cases) was treated with traditional Chinese medicine cycle therapy combined with metformin for 3 months，the control group (45 cases) was treated with metformin treatment for 3 months, the efficacy and safety of the two groups were compared, of the two groups of patients before and after treatment of sex hormones, fasting blood glucose (FPG), fasting insulin (FINS), insulin resistance (HOMA-IR), changes in ovulation rate were analyzed. Results After treatment, the clinical symptom scores of menstrual thinning, amenorrhea, backache, cold, libido, hairy, urination, and stool symptoms in the two groups decreased (P < 0.05), and the clinical symptom scores in study group were lower than the control group (P < 0.05). After treatment, E2, FSH, ovulation rate in two groups increased (P < 0.05), LH, T, LH/FSH, FPG, FINS, HOMA-IR in study group decreased (P < 0.05), LH, FPG, FINS, HOMA-IR in control group decreased (P < 0.05), T, FSH, LH/FSH in control group had no change (P > 0.05). After treatment, the LH, T, LH/FSH, FPG, FINS and HOMA-IR in study were lower than the control group (P < 0.05). The effective rate of treatment in the study group was higher than the control group (P < 0.01), and the incidence rate of adverse reaction was lower than the control group (P < 0.05). Conclusion Chinese medicine cycle therapy combined with metformin in the treatment of normal weight PCOS is more effective, safer, and can significantly reduce the level of sex hormones and insulin resistance in PCOS patients.
