交泰丸联合降糖康复操对2型糖尿病湿热困脾证患者脂联素及胰岛β细胞功能的影响

摘要目的探讨交泰丸联合降糖康复操对2型糖尿病湿热困脾证患者血脂联素及胰岛β细胞功能的影响。方法选择2017年3月～2018年8月南京中医药大学附属中西医结合医院内分泌科收治的符合中西医诊断标准的2型糖尿病湿热困脾证患者300例，采用随机数字表法分为对照组150例和联合组150例。对照组依照原有的药物方案（西药降糖、降压、降脂）治疗，联合组在此基础上予口服交泰丸联合康复操锻炼治疗，疗程均为12周。观察两组治疗前后的中医症候积分、空腹血糖（FBG）、糖化血红蛋白（HbA1c）、血清脂联素（APN）、胰岛β细胞功能指标[胰岛素分泌指数（HOMA-β）、空腹胰岛素（FINS）]的变化及两组临床疗效的比较。结果最终联合组脱落1例。治疗后，两组中医症候积分、FBG、HbA1c水平均下降，APN、HOMA-β、FINS水平均升高（P ＜ 0.05），且联合组治疗后中医症候积分、FBG、HbA1c水平低于对照组，APN、HOMA-β、FINS水平高于对照组（P ＜ 0.05），治疗后联合组的总有效率高于对照组（P ＜ 0.05）。结论交泰丸联合降糖康复操治疗能够有效降低湿热困脾证2型糖尿病患者的血糖水平，改善患者的中医症状，提高胰岛β细胞功能，其机制可能通过提高血脂联素水平相关。

Abstract：Objective To investigate the effects of Jiaotai Pills combined with hypoglycemic rehabilitation on blood adiponectin and β cell function of the pancreas in patients with type 2 diabetes dampness and spleen syndrome. Methods From March 2017 to August 2018, 300 patients with type 2 diabetes with dampness and spleen syndrome who were admitted to the Department of Endocrinology, Affiliated Hospital of Integrated Traditional Chinese and Western Medicine, Nanjing University of Chinese Medicine were selected, Random number table method was used to divide 150 cases into the control group and the combined group, respectively. Control group according to the original drug regimen (Western medicine hypoglycemic, hypohypertensive, lipid-lowering) treatment. The combined group was treated with oral Jiaotai Pills combined with rehabilitation exercise. The course of Treatment was 12 weeks. The changes of TCM syndrome score, fasting blood glucose (FBG), glycosylated hemoglobin(HbA1c), serum adiponectin (APN), islet β function index (HOMA-β), fasting insulin (FINS) before and after treatment in the two groups were observed, and the clinical efficacy was compared between the two groups. Results One case of combined group lossed at lase. After treatment, TCM syndrome scores, FBG, and HbA1c levels were decreased in both groups, and APN, HOMA-β and FINS levels were increased in all the groups (P ＜ 0.05). Moreover, the TCM syndrome score, FBG and HbA1c levels in the combined group were lower than those in the control group, and the APN, HOMA-β and FINS levels were higher than those in the control group (P ＜ 0.05). The total effective rate of the combined treatment group was higher than that of the control group (P ＜ 0.05). Conclusion Jiaotai Pills combined with hypoglycemic rehabilitation surgery can effectively reduce blood sugar levels in patients with type 2 diabetes mellitus, improve the symptoms of TCM and the function of islet β cells. The mechanism may be related to the increase of blood adiponectin levels.
