摘要段光堂教授认为慢性肾衰竭的基本病机为本虚标实，以脾肾虚损为本，湿浊、瘀血壅扰为标。健脾补肾泄浊化瘀法是治疗本病的基本法则。本文主要从病因病机及治则、肾衰Ⅰ号及肾衰Ⅳ号方组方分析、临床应用等几个方面探讨段光堂教授治疗慢性肾衰竭的学术观点及临证经验。临床总结以肾小球疾病为主所致的慢性肾功能衰竭，临床多表现为脾肾气（阳）虚为主；而以小管间质疾病所致的慢性肾功能衰竭，则多表现为肾气（阳）不足为主。采用补益脾肾为主，研制成肾衰Ⅰ号、Ⅳ号方治疗本病临床取得良好疗效，为无数慢性肾衰竭患者延缓了肾功能进展，推迟了进入透析的时间，并附验案二则。

Abstract：Professor Duan Guangtang considers that the basic pathogenesis of chronic renal failure is deficiency in origin and enrichment in symptom. The former is asthenia of the spleen and kidney, the latter is accumulated blood stasis and damp turbidity. The basic rules for the treatment of this disease are invigorating the spleen and kidney, excreting turbid and removing blood stasis. The article introduces Professor Duan Guangtang′s academic viewpoint and clinical experience from the aspects of the pathogenesis and treatment of pruritic dermatosis, the presciption analysis of the NO.1 and NO.4 Decoction, and the clinical application of this method in the treatment of chronic renal failure diseases. Took the chronic kidney failure caused by glomerular disease, the main clinical manifestations included spleen-kidney qi(yang) deficiency; while the chronic kidney failure caused by renal tubular interstitial disease, the main manifestations included kidney-qi(yang) deficiency. Mainly tonifying the spleen and the kidney is adopted, we achieve well curative effect by using NO.1 and NO.4 Decoction to treat this disease, the therapy of Chinese medicine can curb the deterioration progress of renal function for thousand patients, thereby delay the time before entering dialysis therapy. Attachs two proved cases.
