针刺神堂和脑户穴配合重复经颅磁刺激治疗中风后抑郁的效果

摘要目的观察针刺神堂、脑户穴配合重复经颅磁刺激治疗中风后抑郁（PSD）的效果。方法选取2019年1月至2021年12月就诊于石家庄市中医院康复科的PSD患者90例，采用随机数字表法将其分为对照组1、对照组2和观察组，每组30例。对照组1采用针刺神堂、脑户穴进行治疗，对照组2采用重复经颅磁刺激治疗，观察组采用针刺神堂脑户穴配合重复经颅磁刺激治疗。4周为1个疗程，治疗2个疗程后，比较三组治疗前后汉密尔顿抑郁量表（HAMD）评分、美国国立卫生研究院卒中量表（NIHSS）评分、Barthel指数、中医证候积分及血清5-羟色胺、去甲肾上腺素水平的变化情况。结果治疗后，三组HAMD、NIHSS评分及中医症状积分低于治疗前，Barthel指数及血清5-羟色胺、去甲肾上腺素水平均高于治疗前，差异有统计学意义（P＜0.05）。治疗后，观察组HAMD、NIHSS评分及中医症状积分均低于对照组1、2，Barthel指数及血清5-羟色胺、甲肾上腺素水平均高于对照组1、2，差异有统计学意义（P＜0.05）。观察组疗效优于对照组1、2，差异有统计学意义（P＜0.05）。结论针刺神堂、脑户穴配合重复经颅磁刺激治疗PSD效果显著，可改善临床症状，促进患者恢复。

Abstract：Objective To observe the effect of acupuncture at shéntáng and nǎohù points combined with repeated transcranial magnetic stimulation in the treatment of post-stroke depression (PSD). Methods A total of 90 patients with PSD admitted to the Department of Rehabilitation, Shijiazhuang Hospital of Traditional Chinese Medicine from January 2019 to December 2021 were selected and divided into control group 1, control group 2, and observation group by random number table method, with 30 cases in each group. Control group 1 was treated with acupuncture at shéntáng and nǎohù points; control group 2 was treated with repeated transcranial magnetic stimulation; observation group was treated with acupuncture at shéntáng and nǎohù points combined with repeated transcranial magnetic stimulation. Four weeks was a course of treatment, after two courses of treatment, the changes of Hamilton depression scale (HAMD) score, National Institutes of Health stroke scale (NIHSS) score, Barthel index, traditional Chinese medicine syndrome score, and serum levels of 5-hydroxytryptamine, norepinephrine before and after treatment were compared among the three groups. Results After treatment, HAMD, NIHSS score, and traditional Chinese medicine symptom score of three groups were lower than those before treatment, while Barthel index, serum levels of 5-hydroxytryptamine and norepinephrine were higher than those before treatment, the differences were statistically significant (P＜0.05). After treatment, HAMD, NIHSS score, and traditional Chinese medicine symptom score of observation group were lower than those of control group 1 and 2, while Barthel index, serum levels of 5-hydroxytryptamine and norepinephrine were higher than those of control group 1 and 2, the differences were statistically significant (P＜0.05). The curative effect of the observation group was better than those of the control group 1 and 2, and the differences were statistically significant (P＜0.05). Conclusion Acupuncture at shéntáng and nǎohù points combined with repeated transcranial magnetic stimulation has a remarkable effect in the treatment of PSD, which can improve clinical symptoms and promote the recovery of patients.
